Echo Case 69: Echocardiography Spot Diagnosis Series


A 35 year old male with dyspnea. Spot the findings on the basis of echocardiography clips shown, answer is given after thirty seconds with recap of echo clips and explanation of findings.

FINDINGS & DIAGNOSIS: 

Transthoracic Echocardiogram showing subvalvular thickening on PLAX view, further cleared with long axis view on transesophageal echocardiography. Continous Wave doppler profile showing increased velocity across Aortic valve of 3.2 m/s suggesting moderate stenosis. Note the rounded shape of the CW doppler profile typical of fixed obstruction, in dynamic obstruction of Hypertrophic obstructive cardiomyopathy (HOCM), its dagger shapped.
